Filming Consent Form
Everyone filmed in a Life Feature signs this form before the camera rolls. Print this page, sign it, and bring it to your filming day, or simply sign on the day. Nothing starts without it.
1. Consent to be filmed
I consent to being filmed and recorded by Life Feature for the purpose of producing a personal documentary film. I understand the filming will take place at the agreed location(s) on the agreed date(s), and that I may pause or stop filming at any time, for any reason, without explanation.
2. Use of the finished film
I understand the finished film is produced for my family and me for personal, non-commercial use. Life Feature will not publish, broadcast, sell or share the film, or any clip, still or audio from it, with anyone outside my nominated family group without my separate written permission.
3. Optional marketing permission
Where I have agreed (for example, as part of a founding client offer), Life Feature may use one short excerpt of up to 60 seconds from my film for its own portfolio and marketing. I can withdraw this permission at any time by emailing hello@lifefeature.co.uk, and any published excerpt will be removed within 7 days.
4. Archive materials
Any photographs, home videos, letters or documents I provide remain my property (or the property of their rightful owner). I confirm I have the right to share them for inclusion in the film. Originals will be returned to me after production.
5. People appearing with me
If family members or others appear in my film, I will make sure they have seen this form and agree to take part. Anyone appearing in the film has the same right to pause or stop at any time.
6. Sensitive content
I understand I am in control of what I share. Anything I say can be removed from the finished film at my request during the revision stage. I may also declare any topic off the record at any point during filming, and it will not be used.
7. Data protection
My personal data and materials will be handled in line with the Life Feature privacy policy (lifefeature.co.uk/privacy) and UK data protection law.
